These order forms allow you to work out the price of your film transfer There is no obligation to purchase.

INSTRUCTIONS

• Select the form type for your films
• Enter all the details you can
• Print (by pressing on the print button at the bottom of the form)
• Post with films to our address below

 NOTE: if you have more than one type of film simply fill in one form first and print it out then fill in the next type - note, you only have to fill in your personal details on one form)

Now you have printed your form, enclose it with your films and post by recorded or special delivery to our address below. On arrival we will acknowledge the safe receipt of your film and check your order form, if there are any queries we will contact you. No deposit required, payment is requested after the films or photos are transferred.
